The Physician Consent Form needs to be completed by the camper's primary care doctor in order for your Camp Patrick application to be complete. This will need to be done yearly so that our medical staff has the most up to date medical information for your camper. Please download this document and take it to your PCP. Once it is complete, you can then upload it into your Camp Brain Account. All campers attending Camp Patrick must have the Physician Consent Form completed before they can attend camp.
